The FSP by Federal State: Why the Same Exam Is Not the Same Everywhere

Two colleagues with identical training can face different requirements, different documents and different waiting times. Not because one made a mistake, but because they applied in different federal states.

Why there is no single national rule

Under the German constitution, health and education are matters for the federal states rather than the federal government. Each of the sixteen states therefore runs its own recognition procedure through its own authority.

This is not an administrative accident that will be tidied up. It is how the country is built, and it explains almost every contradiction you will find when you read about recognition online. Two accounts can both be accurate and still describe different processes, because they come from different states.

What actually differs between states

Four things vary, and each one can change your plan:

Doctors: the chamber of your state

For doctors the picture is the most consistent. General German at B2 level, plus a specialist language examination set at C1, taken at the medical chamber of the federal state.

The chamber differs, and details of organisation differ with it — where you register, how you are scheduled, how long you wait. The shape of the examination itself is broadly comparable across the country: a patient conversation, written documentation, and a professional conversation.

So for doctors the state changes the logistics more than the substance. You will prepare for the same three tasks wherever you apply.

Health professions: a more varied picture

For nursing, physiotherapy, radiology technology and laboratory technology, the differences are more substantial.

Several states accept a B2 certificate in general German as proof of language for nursing — Bavaria among them. Other states run a specialist language examination for the health professions. Both approaches exist in Germany today, side by side.

In Bavaria, the health professions are handled by the state office for nursing rather than by a professional chamber, which is one reason the procedure feels different from the one doctors describe.

Choosing your state is a real decision

Because so much follows from it, the choice of state deserves more thought than it usually gets. It is worth weighing together:

• Where the work you want actually exists, and where employers in your field are concentrated.
• What the state asks for as proof of language — this decides months of preparation.
• Where you would genuinely want to live, since recognition ties you to a place for the duration of the process.

Once recognition is granted it generally applies across Germany. But the procedure belongs to the state you applied in, so its requirements are the ones that shape your year.

How to find the rule that applies to you

Three steps, in order, and they take less time than reading forums.

1. Name your profession precisely — the German designation, not an approximate translation. Nursing, physiotherapy and the technology professions each have their own path.
2. Find the authority for that profession in your chosen state, and read its own page rather than a summary of it.
3. Write to them with one question: which proof of language do you accept for this profession? Keep the reply.

That written answer is worth more than any general article, including this one. It is the only version of the rule that applies to your file.

And whichever answer comes back, the German you will speak on your first shift is identical: the handover, the patient who needs plain words, the colleague who needs the compressed version. That part is worth building while the paperwork moves.
