Why shortness of breath comes up in the Fachsprachprüfung

Dyspnoea is a leading symptom with many causes — heart, lungs, vessels. That is exactly why the case suits the exam: you show that you ask purposefully and transfer the findings cleanly into medical German.

The Fachsprachprüfung (FSP) tests three situations: the history-taking conversation with the patient, the written documentation (Arztbrief) and the case presentation to a doctor. During history taking you speak to the patient in everyday German — short, clear, without technical terms. Ask in this order and nothing will be missing at the end:

The case presentation: how you report to the senior physician

The case presentation always follows the same order: patient and main complaint, course, accompanying symptoms, previous illnesses, medication, allergies, substance use, family and social history, suspected diagnosis, differential diagnoses, next steps. This is what it sounds like in connected sentences:

Ich stelle Ihnen Frau Krüger vor, 71 Jahre alt, die sich mit seit drei Tagen zunehmender Belastungsdyspnoe vorstellt, inzwischen bereits nach einem Stockwerk. Sie berichtet über Orthopnoe und beidseitige Unterschenkelödeme sowie eine Gewichtszunahme von etwa drei Kilogramm in einer Woche. Husten, Fieber und Thoraxschmerzen werden verneint. An Vorerkrankungen sind eine chronische Herzinsuffizienz, eine arterielle Hypertonie und Vorhofflimmern bekannt. The Arztbrief: how you document the case

In the Arztbrief you write concisely, in nominal style and in medical German. Every section has its fixed place:

Anamnese: Seit 3 Tagen progrediente Belastungsdyspnoe (NYHA III), Orthopnoe, beidseitige Unterschenkelödeme, Gewichtszunahme ca. 3 kg/Woche. Kein Husten, kein Fieber, keine Thoraxschmerzen.

Vorerkrankungen: Chronische Herzinsuffizienz, arterielle Hypertonie, Vorhofflimmern.

Medikation: Torasemid 10 mg 1-0-0 (unregelmäßig eingenommen), Bisoprolol 5 mg 1-0-0, Apixaban 5 mg 1-0-1.

Allergien: Keine bekannt.

Noxen: Nichtraucherin, kein Alkohol.

Sozialanamnese: Lebt allein, bisher selbstständig im Haushalt.

Verdachtsdiagnose: V. a. kardiale Dekompensation bei bekannter Herzinsuffizienz.

Differenzialdiagnosen: Pneumonie, Lungenembolie, COPD-Exazerbation, Pleuraerguss.

Frequently asked questions about this case

How do I ask about the severity of the breathlessness?

Ask about the exertion at which it occurs: “Wie viele Stockwerke schaffen Sie, ohne stehen zu bleiben?” and “Haben Sie die Luftnot auch in Ruhe?” In medical German you then assign the answer to an NYHA class.

Do I have to get the diagnosis right in the FSP?

What is assessed is your German: whether you ask comprehensibly, document precisely and present in a structured way. A plausible suspected diagnosis with fitting differential diagnoses shows you have understood the case — the medical language carries you through all three parts.

How long does each part of the Fachsprachprüfung take?

At most medical chambers (Ärztekammern) each of the three parts — history taking, documentation, case presentation — takes about 20 minutes. The exact procedure is set by the responsible Ärztekammer; its website states the format and the assessment criteria.
