You have the FSP certificate, in Bavaria at B2 level. Now the recognition office decides on your certificate as a radiographer. Here you read what goes into the file, what a deficit decision means and which German is used in the department.
What your certificate proves
The FSP proves that you act safely in German in the department: informing and preparing the patient, handover and questions in the team, and written documentation. In Bavaria it is taken for MT-R at B2; other states check professional language inside the adaptation or accept a general B2 certificate.
Since the law on technical health professions of 2023 your profession is called MTR and covers diagnostic radiology, radiotherapy and nuclear medicine. Your certificate is one part of the file; the authorisation comes from the recognition office.

The file: what goes to the authority now
The recognition office of your federal state decides: depending on the state a state examination office for health professions, a regional council, a state office for social affairs or a district government. The file usually holds:
- your certificate and training record with hours of theory and practice, certified and translated,
- your language proof, in Bavaria the FSP certificate,
- your professional experience and references, with the devices and modalities you work with,
- a certificate of good conduct and a medical statement of fitness,
- your radiation protection courses, if you have them.
The office compares your training with the German one. If content and hours match, the certificate arrives. If it sees differences, a decision arrives naming the missing areas.
The decision: adaptation or knowledge test
Two routes stand in the decision:
- Adaptation measure: supervised practice in the department, plus theory in the missing areas, with a final talk. It often covers radiation protection, quality assurance and documentation under German rules.
- Knowledge test: an examination on the material of the German training, practical and oral, at the device and on a case.
Many employers hire you during the adaptation already. Before you sign, ask who guides you, on which modalities you will be trained and how the theory hours sit in the shift plan.
From the certificate to the contract
With the certificate you are graded as an MTR: public employers usually apply the sector agreement, university hospitals the agreement of the federal state. It sets the pay group, the steps by years of practice and the allowances for on-call, night and weekend work.
Two questions are worth asking in the interview: which devices you will work with and what the induction per modality looks like, and which further training the employer pays for, for example MRI, CT or radiotherapy.
Your German in patient information and documentation
Three situations shape your day:
- Information and preparation: explain the course, the duration, the noises and the positioning, in words that arrive.
- Questions and handover: name contraindications, contrast agents and anything unusual precisely.
- Documentation: record the examination, the parameters and any particularities completely.
Those three are what the professional language course trains: real cases from the department, conversation simulators and immediate correction of your sentences.
