
Your language proof is in place. Now the recognition office decides on your certificate as a physiotherapist. Here you read what goes into the file, what a deficit decision means and which German is used in the first session, in the assessment and in the report.
What your language proof shows
For physiotherapy the language level in recognition is B2. Bavaria additionally asks for an official German exam for the profession at B2: a first session with the patient, a professional conversation with a colleague or a doctor and a written documentation. Other states check professional language inside the adaptation or accept a general certificate.
Your proof is one part of the file. The authorisation to work comes from the recognition office, after it has compared your training with the German one.

The file: what goes to the authority now
The recognition office of your federal state decides: depending on the state a state examination office for health professions, a regional council, a state office for social affairs or a district government. The file usually holds:
- your certificate and training record with hours of theory and practice, certified and translated,
- your language proof, in Bavaria the certificate of the professional language exam,
- your professional experience and references,
- a certificate of good conduct and a medical statement of fitness,
- your CV and registration certificate, if you are already in Germany.
One point belongs to physiotherapy: in Germany you treat on a medical prescription (Heilmittelverordnung), with diagnosis, therapy goal and number of sessions. The office therefore looks closely at assessment, documentation and your work with the doctor.
The decision: adaptation course or knowledge test
If the office sees differences, a deficit decision arrives with two routes:
- Adaptation course: supervised practice in a facility, plus theory in the missing areas, with a final talk.
- Knowledge test: an examination on the material of the German training, practical and oral, often with assessment and treatment plan on a case.
Many practices and rehabilitation centres hire you during the adaptation already. Before you sign, ask who guides you and how the theory hours sit in the plan.
From the certificate to a post in a practice or hospital
With the certificate you apply in a practice, in a rehabilitation clinic or in a hospital. Public employers usually apply the sector agreement; practices have their own contracts. In the interview your areas count, for example orthopaedics, neurology or respiratory therapy, and your experience with documentation.
Two questions are worth asking: which certificate qualifications the employer supports, for example manual therapy, lymphatic drainage or Bobath, and how much time per session the plan allows.
Your German in assessment and treatment
Three situations shape your day:
- First session and assessment: ask, sort what you hear, agree on a first goal.
- Guiding the treatment: announce every movement, in words that arrive.
- Report and feedback to the doctor: course, goal and recommendation, written factually.
