Why abdominal pain comes up in the Fachsprachprüfung
Abdominal pain calls for a precise pain history and — in female patients — the gynaecological history. In the exam you show that you ask sensitive questions respectfully and still completely.
The Fachsprachprüfung (FSP) tests three situations: the history-taking conversation with the patient, the written documentation (Arztbrief) and the case presentation to a doctor. This article works through the case from start to finish — with the German sentences you need in each of the three parts.
History taking: the questions you ask
During history taking you speak to the patient in everyday German — short, clear, without technical terms. Ask in this order and nothing will be missing at the end:

The case presentation: how you report to the senior physician
The case presentation always follows the same order: patient and main complaint, course, accompanying symptoms, previous illnesses, medication, allergies, substance use, family and social history, suspected diagnosis, differential diagnoses, next steps. This is what it sounds like in connected sentences:
Ich stelle Ihnen Frau Yilmaz vor, 34 Jahre alt, die sich mit seit gestern bestehenden Schmerzen im rechten Unterbauch vorstellt. Die Schmerzen begannen periumbilikal und wanderten im Verlauf in den rechten Unterbauch; sie sind dauerhaft, die Intensität liegt bei acht von zehn und nimmt bei Bewegung zu. The Arztbrief: how you document the case
In the Arztbrief you write concisely, in nominal style and in medical German. Every section has its fixed place:
Anamnese: Seit gestern Schmerzen im rechten Unterbauch (NRS 8/10), initial periumbilikal, im Verlauf in den rechten Unterbauch wandernd, Zunahme bei Bewegung. Begleitend Nausea, einmalige Emesis, Inappetenz, subfebrile Temperaturen.
Gynäkologische Anamnese: Letzte Menstruation vor 3 Wochen, regelrecht. Schwangerschaft laut Patientin unwahrscheinlich.
Vorerkrankungen / Voroperationen: Keine.

How do I ask respectfully about pregnancy and menstruation?
Announce the questions briefly: “Ich muss Ihnen jetzt ein paar Fragen zur Regelblutung stellen — das ist bei Bauchschmerzen wichtig.” Then ask directly: “Wann war Ihre letzte Regelblutung?” and “Besteht die Möglichkeit einer Schwangerschaft?”
Do I have to get the diagnosis right in the FSP?
What is assessed is your German: whether you ask comprehensibly, document precisely and present in a structured way. A plausible suspected diagnosis with fitting differential diagnoses shows you have understood the case — the medical language carries you through all three parts.
How long does each part of the Fachsprachprüfung take?
At most medical chambers (Ärztekammern) each of the three parts — history taking, documentation, case presentation — takes about 20 minutes. The exact procedure is set by the responsible Ärztekammer; its website states the format and the assessment criteria.
