Your colleague is continuing care and needs your observations, the agreed goal and the patient’s progress. Professional language —Fachsprache in German— helps organise that information. Here you practise an assessment summary and handover that connect findings, goals and the next step.
Identify the source of each piece of information
Make clear who provides the information. Die Patientin berichtet introduces the patient’s account; bei der Beobachtung zeigte sich introduces your observation. Then identify the activity or function concerned.
These are fictional German-practice examples. Complete them with information from your exercise. In professional records, write according to your actual assessment and your institution’s documentation procedures.
Connect assessment, goals and progress
Organise the summary in four steps: reason, relevant information, agreed goal and current progress. Select the terminology this case needs. An observation is easier to understand when you explain how it relates to the activity.
Practise orally first, then in writing. You develop wording for handovers and reports. Choose verbs such as berichten, beobachten, beschreiben, vereinbaren and dokumentieren according to the information you present.
End the handover with an agreement
After the summary, leave space for questions. Respond to the specific point and clarify the information your colleague needs for the next task.
For practice, a partner plays the colleague and asks about a detail. Answer, summarise the agreement and explain the next step. You can adapt this exchange to different situations in a practice, clinic or rehabilitation setting.
